What to bring to your appointment
Please bring the following items to your initial appointment with your Endocrinologist:
A current, valid GP Referral
Copies of any relevant diagnostics, scans & reports (X-rays/imaging/pathology reports, DXA)
Your Medicare card
Private Hospital Insurance information
A list of any medications, vitamins and any over the counter supplements you are currently taking and their dosage
All new patient referrals are reviewed and triaged by our specialists before an appointment is offered, helping us ensure that each patient is seen by the most appropriate specialist and within an appropriate timeframe. Please email your referral to our reception team, who will arrange for it to be reviewed.

Do I need a referral for Evolve Endocrinology?
Yes, you will require a referral from your GP or another specialist. This will allow you to obtain your Medicare rebate. It also provides important background on your health ahead of your consultation.

Is there parking at the Evolve rooms?
The practice is conveniently located in Brighton, with street parking available directly outside and in the surrounding streets. It is also easily accessible by public transport, approximately a five-minute walk from North Brighton railway station.
Are the rooms wheelchair accessible?
Yes, the rooms are wheelchair accessible.
Do I need to pay for my consultation at the time of my appointment?
We kindly ask that payment is made on the day of your appointment. Our preferred payment method is Eftpos and Credit Card.
